Referee Consent Form: For Collection, Use and Disclosure of Personal Information

Respecting the privacy of your personal information is important to Justplay, and taking steps to protect your personal information is required under existing privacy legislation. In compliance with these requirements, Justplay has adopted a Privacy Policy, and has assigned Elaine Raakman to act as the Privacy Information Officer. All Justplay staff members who come in contact with your personal information are aware of the sensitive nature of the information that you have disclosed to us. They are all trained in the appropriate uses and protection of your information.

By agreeing to the contents of this Referee Consent Form, you have agreed that our organization has protocols in place to ensure that:

	Only necessary information is collected about you;
	We will only share your information with your consent;
	The storage, retention and destruction of your personal information complies with existing legislation, and privacy protection protocols;
	Our privacy protocols comply with privacy legislation.

How Justplay Collects, Uses and Discloses Referee Personal Information

Justplay Sport Services Inc. understands the importance of protecting your personal information. To help you understand how we are doing that, we have outlined here how Justplay is using and disclosing your information.

Justplay will collect, use and disclose information about you for the following purposes:

	To deliver efficient service
	To enable us to contact you
	To establish and maintain communication with you and update your personal information
	To generate reports for the Justplay Behaviour Management Program
	To comply generally with the law

By agreeing to the content of this Referee Consent Form, you have agreed that you have given your informed consent to the collection, use and/or disclosure of your personal information for the purposes that are listed. If a new purpose arises for the use and/or disclosure of your personal information, we will seek your approval in advance. You may withdraw your consent for use or disclosure of your personal information, and we will explain the ramifications of that decision, and the process.

By checking the Checkbox at the bottom of the Justplay New User Registration Page (http://www.wejustplay.com/registration/) beside the text; "I have read and I agree to all the Terms & Conditions in the Referee Consent Form", I agree and confirm that I have reviewed the above information that explains how Justplay will use my personal information, and the steps Justplay is taking to protect my information. I know that Justplay has a Privacy Policy, and I can ask to see the Policy at any time.

I agree that Justplay Sport Services Inc. can collect, use and disclose personal information about me as set out above in the information about the organization's privacy policies.
